The Position: ACO Coordinator
Employment Status: Full Time
Hours per two-week pay-period: 72
Essential Job Functions:
- Manage and monitor ACO quality measures and requirements in collaboration with care teams to ensure patients are identified and enrolled in care management.
- Cultivate and coordinate partnerships with patients and care teams to effectively identify and monitor patient care.
- Collaborate with healthcare providers, social workers, etc., to ensure patients receive comprehensive and coordinated care.
Minimum Qualifications:
- Previous healthcare experience
Certifications and Licenses
- Valid Montana Driver's License
- CPR
Position Summary:
Sidney Health Center is seeking an ACO Coordinator to join our I CARE team. The ACO Coordinator is responsible for providing coordination of the multi-faceted operation of the ACO Measures. The coordinator is responsible for managing the ACO quality measures and collaborating with care teams to ensure patients are identified and enrolled in care management to support them reaching their highest level of health and wellbeing. Essential duties of the position include excellent people skills, organizational skills, and the ability to work well with all staff. The ACO Care Management team is critical to providing the highest level of health and wellbeing.
