The Position: Care Management Nurse RN/LPN/MA
Employment Status: Full Time
Hours per two-week pay-period: 72
Essential Job Functions:
- Develop, implement and monitor personalized care plans for clients ensuring all services are aligned with their health and wellness goals
- Serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing concerns.
- Collaborate with healthcare providers, social workers, etc., to ensure clients receive comprehensive and coordinated care.
- Maintain and management ACO Quality Measure Requirements
Minimum Qualifications:
- Valid Montana Nursing License RN or LPN if applicable
- Previous experience in caring for patients preferred
Certifications and Licenses
- Valid Montana Driver's License
- CPR
Position Summary:
Sidney Health Center is seeking a Care Management RN/LPN/MA to join our I CARE team. The Care Management Nurse will work in collaboration with primary care clinicians and healthcare teams. They act as service liaison, ensuring patients receive comprehensive and coordinated care. Coordinates and supports the management of chronically ill patients through Chronic Care Management, utilizing health coaching techniques to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk. Essential duties of the position include excellent people skills, organizational skills, and the ability to work well with all staff. The ACO Care Management team is critical to providing the highest level of health and wellbeing.
