The Position: Population Health RN or LPN

Employment Status: Full Time

Hours per two-week pay-period: 72

Essential Job Functions:

  • Provide compassionate nursing care services to patients.
  • Assist and support patients with daily care.
  • Perform patient visits and monitor conditions.

Minimum Qualifications:

  • Graduated from an accredited school of nursing.
  • Valid Montana Nursing License RN or LPN or current temporary license
  • Previous experience in caring for patients with chronic disease.

Certifications and Licenses

  • Valid Montana Driver's License
  • CPR

Position Summary:

Sidney Health Center is seeking a Population Health RN or LPN to join our I CARE team in the ACO/Clinic setting. The Population Health nurse will work in collaboration with primary care

clinicians and healthcare teams. They will effectively coordinate and manage high-risk, chronically ill patients through Chronic Care Management, utilizing health coaching techniques to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk. Essential duties of the position include excellent people skills, organizational skills, and the ability to work well with all staff.
